Group Lessons
This is the Group Lesson Registration Form. You can print out the form and turn it in at the front desk, you can fax it in, or you can mail it to us. Please make sure to include your child’s color level and any dates that you wish not to schedule. Payment must be included at the bottom of the form in order to process the registration and hold your child’s spot.

Private Lessons
This is the Private Lesson Request Form. You can print out the form and turn it in at the front desk, you can fax it in, or you can mail it to us. Please fill out more than one option for your day and time availability. Also, don’t forget to include the applicant’s age and what you would like the instructor to focus on during the lesson.

Parent Tot Lessons / Adult lessons
This is the Parent Tot / Adult lessons Registration form. You can print this form out online and either fax it to our center, mail it in, or drop it off in person at the front desk.
